Novant Health Heart and Wellness at Forsyth Medical Center includes the Anticoagulation Clinic and the Heart Failure Program.
The Anticoagulation Clinic exists to increase the time a patient is in therapeutic range and minimize complications associated with anticoagulation therapy by use of point-of-care testing, patient-self- testing, and face-to-face counseling with an expert clinician.
The Heart Failure (HF) Program exists to enhance the quality of life of patients diagnosed with heart failure or cardiomyopathies in the inpatient and outpatient setting by providing state of the art and evidence based comprehensive treatment coordinated to meet the needs of each patient and family across the continuum of care. The program is designed to supplement physician care, with a focus on intense outpatient follow-up, and provision of knowledge and skills necessary for a heart failure patient to self-manage their condition and health. The Heart Failure program is not designed to replace the patient's primary care or specialty physician or to provide traditional primary care services. The Heart Failure Program serves the adult heart failure patient population of all ethnic backgrounds.
